Complaints Form
This form should be used if you wish to make a formal complaint because you believe we have failed to meet our commitments.
Once you have completed it, we will investigate the validity of your complaint and you will receive the outcome of our assessment within 30 days at the email address provided here. If your complaint is upheld, we will take all necessary steps to resolve the issue and, where appropriate, issue a refund and/or credit for any overpayments. If your complaint is rejected, we will provide you with the reasons why.
How to complete the form
All fields on the form are mandatory. It is important to provide your personal and contact details, as well as the type of service to which the complaint relates and the reason for the complaint, including a brief description, and to attach any relevant supporting documents.
